During the early design coordination phase of the primary healthcare center expansion, the structural engineering team raised a critical query regarding the load capacity of the transfer slabs on Level 2. The inquiry was placed as a digital markup on Sheet S-202, specifically questioning whether the reinforcement layout could support the heavy diagnostic imaging equipment scheduled for installation. This comment was flagged as an open item requiring a response from the lead architect and the equipment vendor.

As the project progressed to Revision 3, the document control team consolidated all active markups. A senior coordinator, attempting to clear the review log before the weekly deadline, changed the status of the structural slab comment from 'Open' to 'Resolved'. The coordinator assumed that because the architectural drawings now showed the correct equipment footprint, the structural capacity issues had been coordinated and resolved. A new set of drawings was published without the outstanding structural query, leaving the team under the impression that the slab design was fully approved.

The contractor received conflicting signals: the equipment layouts were marked as finalized, yet the structural drawings lacked the necessary thickening and reinforcement details to support the load, creating a severe structural risk.


Unresolved Issue

Because the digital comment had been prematurely marked as closed, it no longer appeared on the active clash detection reports or the pending design review registers. The critical question of whether the transfer slab could safely support the diagnostic equipment went completely unanswered. The mismatch remained hidden within the design set, bypassing the standard quality control filters and threatening to cause slab cracking or failure upon equipment delivery.

Decision Owner

The Lead Structural Engineer was designated as the primary Decision Owner for this case. In coordination with the Project Management Office (PMO), the owner initiated a full audit of all closed markups to identify why the query was bypassed. They collaborated with the Lead Architect and the Equipment Vendor to re-evaluate the slab design and define clear verification protocols for closing future coordination markups.


Review Outcome

The review resulted in the immediate reopening and formal resolution of the slab markup. The structural team redesigned the Level 2 transfer slab, adding localized carbon-fiber reinforcement and updating the structural schedule on Sheet S-202. To prevent a recurrence, the project team updated their digital markup policy: comments can now only be marked as 'Resolved' or 'Closed' by the discipline that originally authored the query, rather than the document control staff.
